Long-Term Conditions

A long-term Condition is a medical condition that cannot be cured, but symptoms and complications can be managed with regular reviews and treatment. Examples of long-term conditions include:

  • Arthritis
  • Asthma/COPD and certain other lung conditions
  • Diabetes
  • Epilepsy
  • High blood pressure (hypertension)
  • Heart disease

Care And Support Planning

Care and Support Planning is a method of reviewing patients with long-term health conditions. These include:

  • Diabetes
  • High blood pressure (hypertension)
  • Heart disease or any other heart conditions for which you are on long-term medication
  • Asthma/COPD

These appointments could involve a 30 minute appointment with a nurse to take some assessments and blood samples. This is then followed up by a review (which could be 10, 20 or 30 minutes) with a clinician. The review could be face to face, or if the results of the assessments and blood samples are very good, it could be a phone or video consultation.

The aim of Care and Support Planning is to allow you time with the clinician to cover any other factors that affect your condition. This will range from the condition itself to diet and exercise limitations and how it affects your mental health and daily living. You should feel free to mention anything you wish to at either of these appointments and our clinicians will do their best to support you to lead a healther and longer life.
