Register as New Patient
You can register with the Practice if you live within our practice area (see below) and are a permanent UK resident.
Our receptionist will ask you to complete a registration form. If you have your NHS medical card please bring this along. This contains useful information which will enable us to get your medical records to the doctor as quickly as possible.
You will be offered an appointment with a practice nurse for a New Patient Check which is a chance for you to have a health check including blood pressure. If you are already on medication, you can also make an appointment with a doctor to discuss this and set up repeat prescriptions.
You will be registered with a specified doctor but you have the choice of seeing any doctor in the practice.
Meadowcroft Surgery Inner Boundary / Primary Boundary - This is the BLUE AREA. In order to register with Meadowcroft Surgery patients must live in this boundary area.
Meadowcroft Surgery Outer Boundary / Secondary Boundary - This is the ORANGE AREA. Current patients can remain registered with us if their new address is within the Outer Boundary / Secondary Boundary with the exception of patients moving to care homes as we ask they register with the GP Practice/s that regularly visit that care home to benefit from the continuity of care that this offers..

Named Accountable GP
Since 1st April 2015, practices have been required under the GMS contract, to allocate a named, accountable GP to all patients, including children. All patients registered at our surgery are allocated to a named GP. Please ask your doctor, or the reception staff, who your named GP is. Should you express a wish for this to be changed to another GP we will do our best to accommodate your wishes.
Please be aware that this does not affect your ability to make an appointment with any of the GPs in the practice of your choosing.
Change of Details
It is very important we are able to contact you so if you move house or change your name or telephone number please let us know by filling in our change of details form at reception.
Please be aware that we will not take change of details over the phone.
Temporary Patient Registrations
If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.

Disabled Patient Facilities
Disabled patient facilities are also available at this practice.
This includes car park space, toilet facilites, automatic (push button) doors and a ramp.
